Declaration
of provision of consent to the usage of an element of one’s private image – voice, and specifically recorded speech – for the purposes of the “Called by Name” exhibition, which is being held at Dom Bez Kantów, 11 Krakowskie Przedmieście Street, 00-071 Warsaw.
By accepting the present declaration, you provide your consent to the usage of a recording of your voice, this in the form of the name and surname – read out by yourself – of a person commemorated under the “Called by Name” project, by the Witold Pilecki Institute of Solidarity and Valor with its seat in Warsaw at 17 Foksal Street, 00-372 Warsaw, a research institute entered in the Register of Entrepreneurs of the National Court Register kept by the Regional Court for the Capital City of Warsaw in Warsaw, XIIth Economic Affairs Division, under KRS number 0000713483 (hereinafter referred to as “the Institute”), holder of NIP tax identification number 5252735962 and of REGON statistical identification number 369236544, on the following terms and conditions:

The recording of your voice may be used repeatedly and distributed free of charge, among others pursuant to the provisions of Article 6, Subparagraph 1, Letter a of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation – the GDPR Regulation – Journal of Laws of the EU L 119 of 4 May 2016); that is, you provide your consent to the processing of your personal data, and also of your image (made available upon your own initiative).

You may withdraw your consent, granted hereabove, at any time, this by sending a letter to the following address: Instytut Solidarności i Męstwa im. Witolda Pileckiego, ul. Foksal 17, 00-372 Warszawa, or an e-mail to the address of the Data Protection Officer, i.e., iodo@instytutpileckiego.pl.

The recording of your voice shall be utilized for a period of five years for the purposes of the exhibition “Called by Name”, which is being held at Dom Bez Kantów, 11 Krakowskie Przedmieście Street, 00-071 Warsaw.

The personal data obtained from yourself – name, surname, and recorded speech – shall be processed for the purpose of playing back your voice at the exhibition, this in such a way that it shall be heard by persons visiting the said exhibition.

The personal data obtained from yourself may be passed on to:
- data processors processing personal data upon our instruction, and
- public bodies and subjects authorized to obtain personal data in accordance with currently valid legal provisions, for example, courts of law, law enforcement agencies, or state institutions, whenever these so request pursuant to a relevant legal basis.

Your personal data shall not be transferred to third countries.

Furthermore, we would like to inform that you are entitled to:
- access your personal data,
- request the correction or supplementation of your personal data, if the said are inaccurate or incomplete respectively,
- request the removal of your personal data, in particular if you withdraw your consent to processing, when there is no other legal basis for processing,
- request that the processing of your personal data be limited,
- lodge an objection against the processing of your personal data, this on the grounds of your particular circumstances, in instances where we conduct the processing of your personal data on the basis of our legitimate interests or for the purposes of direct marketing,
- transfer your personal data,
- lodge a complaint with the personal data protection supervisory authority, i.e., the President of the (Polish) Personal Data Protection Office.

To the extent that your data are processed on the basis of consent provided by myself, you shall be entitled to withdraw such consent at any time. Your withdrawal of consent shall not affect the legality of processing performed on the basis thereof prior to said withdrawal. You may withdraw your consent by sending a declaration informing of withdrawal of consent to either our regular mail correspondence address or e-mail address.

We further inform that we do not employ automated decision-making systems.

By accepting the present declaration, you provide your consent to the possible dissemination of the recording of your voice in informational, promotional and advertising materials of the Institute, and also in media materials developed by other institutions.
